Noble Women’s Healthcare is closed. If you need to obtain your records, this site is the only option. Please complete the Release of Information on the forms page and email it back to me on the site. Be aware that to copy records less than 7 pages, there is no charge. For 8-10 pages, there is a charge of $15.00. For 11-50 pages, the charge is $0.50 per page and beyond 51 pages, it is $0.25 per page. You will need to pay in advance of the release of records. You can email me to confirm the number of pages in your record. Payment should be mailed to P. O. Box 1066, Corvallis, Oregon 97333.
